Washington state currently allows direct access to physical therapy services. Low Back Habit PLLC provides services on an out-of-network and direct pay basis. If you choose to receive services without a referral, your insurance may not cover these services. Clients are ultimately responsible for the payment of services rendered. By continuing, you acknowledge that you are aware of this direct access policy.
6. Limitations of Liability
Low Back Habit PLLC is not liable for any damages arising from the use or inability to use the Website, including but not limited to errors, omissions, or inaccuracies in the content. You agree that your use of the Website is at your own risk.
7. Cancellation and No-Show Policy
We are committed to providing high-quality care and respecting your time. For this reason, we ask that clients provide at least 24 hours’ notice for any cancellations or appointment changes. Cancellations or missed appointments without 24-hour notice will incur a cancellation fee of $149, which will be charged to the card on file. After three cancellations or no-shows, Low Back Habit PLLC reserves the right to limit you to same-day appointments only.
